Commonwealth Counseling Agency provides structured, person-centered behavioral health services designed to support individuals as they work toward greater stability, improved coping skills, and overall well-being. Our team is committed to providing compassionate care in a supportive environment while helping each individual work toward meaningful treatment goals.  

Commonwealth Counseling Agency's Partial Hospitalization Program (PHP) provides structured behavioral health treatment for individuals who need a higher level of support while remaining engaged in their community. Our program provides a therapeutic environment focused on stabilization, skill development, recovery, and helping individuals make meaningful progress toward their treatment goals. 

• Psychoeducation — Education designed to increase understanding of behavioral health, recovery, and healthy decision-making. 
• Process Groups — A supportive environment where individuals can explore experiences, challenges, progress, and recovery goals. 
• Coping & Life Skills — Practical strategies for managing stress, emotions, triggers, communication, and everyday challenges. 
• Individualized Treatment Planning — Treatment goals developed around each individual's assessed needs and progress. 
•  Case Management — Individualized support to help clients identify needs, address barriers to treatment, access community resources, coordinate services, and develop plans that support continued stability and recovery. 
• Care Coordination — Support connecting individuals with appropriate community resources and ongoing services. 
• Recovery-Focused Support — Building skills and supports that encourage stability, personal growth, and continued recovery.

Commonwealth Counseling Agency’s Partial Hospitalization Program may be appropriate for adults who need structured behavioral health support while continuing to live within the community.

PHP may be appropriate for individuals who:

• Need more structure and support than traditional outpatient services can provide. 
• Are experiencing behavioral health or substance use challenges that are affecting daily functioning. 
• Would benefit from frequent therapeutic groups, skill development, and structured support. 
• Need assistance developing healthier coping strategies and managing triggers, stress, or emotional challenges. 
• Would benefit from case management, care coordination, and connection to community resources. 
• Are transitioning from a higher level of care and need continued structured support. 
• Can safely participate in treatment without 24-hour inpatient or residential supervision.
