Taking the first step toward receiving support should feel simple and straightforward. Commonwealth Counseling Agency is here to guide you through the admissions process, answer your questions, and help determine whether our services are appropriate for your needs.

How to Get Started

 

1. Contact Our Team
Reach out to Commonwealth Counseling Agency to begin the admissions process. Individuals, family members, and professional referral sources may contact our team.


2. Provide Basic Information
Our team will collect basic demographic, contact, insurance, and referral information needed to begin the process.


3. Insurance Verification
We will verify applicable insurance coverage and benefits and provide information regarding the next steps.

 4. Complete an Assessment
An assessment will be completed to better understand the individual's needs and determine whether CCA's program is an appropriate level of care.


5. Admission & Start Date
If admission is appropriate, our team will complete the necessary paperwork, review program expectations, and establish a start date.

What You’ll Need

  •  Photo Identification — A valid government-issued photo ID, if available. 
  • Insurance Information — Your current insurance card and policy information. 
  • Current Medications — A list of medications you are currently taking, including dosage when known. 
  • Treatment Information — Information regarding current or previous behavioral health or substance use treatment, when applicable. 
  • Referral Documentation — Any referral paperwork provided by another treatment provider, hospital, physician, or community agency. 
  • Legal or Supervision Documentation — Relevant probation, parole, pretrial, court, or other documentation when services are connected to a legal referral. 
  • Emergency Contact — Name and contact information for an emergency contact.

Insurance & Payment

 Commonwealth Counseling Agency understands that navigating insurance and treatment costs can feel overwhelming. Our team is available to help you understand the insurance verification process before services begin. 

 Insurance Verification

Before admission, our team will verify your insurance coverage and applicable behavioral health benefits. Verification of benefits does not guarantee coverage or payment. Final coverage is determined by your insurance plan based on eligibility, authorization requirements, medical necessity, and other plan guidelines.

Professional Referrals

Commonwealth Counseling Agency welcomes referrals from healthcare providers, behavioral health professionals, hospitals, community organizations, criminal justice partners, and other agencies seeking appropriate behavioral health services for the individuals they serve.

Refer an Individual

Our admissions team will work with the referring professional and prospective client to help coordinate the admissions process, verify applicable insurance benefits, complete the assessment process, and determine whether our program is an appropriate level of care.

 Referral Information May Include:

• Client name and contact information
• Date of birth
• Insurance information
• Reason for referral
• Relevant treatment history
• Current medications, when applicable
• Probation, parole, pretrial, or court information, when applicable
• Relevant clinical or referral documentation
